Most embarassing VA moment for me, had to be about 4 years ago.
I lived at home at the time- so I usually recorded when my Aunt left the house with the rest of my family, and I was recording lines for one character, for an abridged character (I think its been a while lmao), who had to shout,"To get a man to give you what you want, all you have to do, is show them your boobs!" I got really into the take, taking about 15 different variations... I was pleasantly surprised when the door burst open, revealing my aunt and uncle looking at me in utter shock and concern.
I was in a live call that day, and had to try and explain to my aunt and uncle who raised me what was going on...Breaking the new to them was so awkward and embarassing- to this day it still makes me cringe.

One time an animator asked me to voice in an animation of his as he thought I'd be able to do an accurate impression of a specific character but when we got into the live call to record; the character's voice just wasn't all that replicable for someone with a voice like mine so it resulted in a pretty awkward I'm gonna have to cast someone else moment during the recording. Only time I've ever really been recast.

So, I used Discord for voice over typically, so one day my parents told me I couldn’t use Discord because there was to mcuh swearing.
So then I had to find ways to chat with every single client.
Every. Single. One.
Not to mention A LOT of my roles went down the drain because of that. Fun times.
